HOURS OF OPERATION*:
Monday - Friday: 7am - 6pm

Saturday: 9am - 5pm - Sunday: 11am - 5pm

Please have the following information ready:

• Location and details of Pick-Up

• Location and details of Drop-off

• Is this a Round-Trip or One-Way? (If One-Way, is this a Discharge?)

• Type of Service - Ambulatory, Wheelchair, Stretcher

(if Wheelchair, do we need to bring one?)

• Patient/Rider First & Last Name

• Patient/Rider Date Of Birth

• Will they have an escort riding along (we allow 1 to ride for free)

• Name of person placing the order

• Facility or person responsible for payment
